Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU AND YOUR CHILD MAY BE USED AND DISCLOSED, AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our Commitment to Your Privacy

We are required by law to maintain the privacy of your and your child's protected health information ("PHI"), provide this Notice describing our legal duties and privacy practices, and follow the terms of the Notice currently in effect.

How We May Use and Disclose Health Information

Treatment. We may use and share PHI to provide, coordinate, or manage your and your baby's care — for example, discussing your feeding plan, coordinating with a referral provider, or documenting a visit using our clinical documentation tools (currently Heidi AI and Freed AI), as described in your AI Documentation Consent.

Payment. We may use and share PHI to bill for services, including submitting information to your insurance carrier if you use our insurance-covered concierge pathway, and to obtain payment for care provided.

Healthcare Operations. We may use PHI for activities such as quality review, care coordination, and business planning.

Other permitted or required uses may include: as required by law; to prevent a serious threat to health or safety; for public health activities; in response to a court order or legal process; or other purposes specifically permitted under HIPAA.

Secure communication. If you consent to secure messaging (currently through Spruce), text, phone, or email as part of your care, PHI may be shared through those channels consistent with your Electronic Communication Consent.

Any other use or disclosure not described above requires your written authorization, which you may revoke in writing at any time, except to the extent we've already relied on it.

Your Rights

  • Inspect and receive a copy of PHI we maintain about you and your child, with limited exceptions

  • Request an amendment if you believe your PHI is incomplete or incorrect

  • Request an accounting of certain disclosures we've made of your PHI

  • Request restrictions on certain uses or disclosures (we're not required to agree, except in limited circumstances)

  • Request confidential communications — for example, a specific phone number or email for contact

  • Receive a paper copy of this Notice at any time, even if you agreed to receive it electronically

  • File a complaint if you believe your privacy rights have been violated, either with us directly or with the U.S. Department of Health and Human Services, Office for Civil Rights. Filing a complaint will not affect your care.

Our Responsibilities

We're required to maintain the privacy and security of your PHI, notify you if a breach occurs that may have compromised your information, and follow this Notice's terms. We may change these terms; any change applies to PHI we already have as well as PHI we receive in the future, and an updated Notice will be made available to you.
